Informed consent is the responsibility of the surgeon. Surgeon must explain risks and benefits of the the surgery he or she is performing before patient signs informed consent
No sedation should be given to client prior to signing consent
Older clients or clients who have dementia or are confused may need legal guardian to sign
Minors must have parents signed unless emancipated minors
The nurse may be a witness after she or he makes sure that client understands procedure.
