An 80-year-old male client with a history of congestive heart failure and who lives in a nursing home is admitted to the emergency department because of weakness. The wife states that due to the COVID pandemic, she has not been able to visit daily but that the nurses reported that the client has been progressively weak for the last 3 days, has not been eating well but has been drinking fluids, and has had 3-4 loose stools daily. The client is lethargic, confused, and reports seeing yellow halos.
Current medication include digoxin 0.25 mg po daily, captopril 25mg po daily, furosemide 40 mg po daily, famotidine 40mg po BID, and colace 250 mg po daily.
Vital signs: Temp 98.6 °F, BP 99/55, HR 50, RR 16. Physical assessment reveals 2+ edema to both lower extremities, JVD, and hepatomegaly. Head CT: Negative
