Multiple Myeloma

A malignant proliferation of plasma cells in the bone. Malignant plasma cells produce Bence Jones protein found in the blood and urine. Treatment is chemotherapy, radiation, steroids, and plasmapheresis (removal of immunoglobulins or abnormal antibodies)

Multiple Myeloma and the Kidneys

Hypercalcemia, hyperuricemia, and proteinuria cause damage to kidneys

Hydration

> 2L per day

Furosemide (diuretic)

To promote excretion of calcium

IV fluids

To protect kidneys & ↓ Calcium

Prevent Fractures

Bones lose calcium (calcium moves into the serum.  This is where hypercalcemia comes from). Bones become weaker.

Safe Environment

to prevent falls. Encourage mobility

Bisphosphonates

Such as pamidroate andzoledronic acid to slow down bone damamge and ↓ fractures
