- Verify the client’s identifiers with another LICENSED Nurse.
- Best IV: 18 gauge to prevent hemolysis
- Only 0.9% NaCL can be infused with blood products n
- Must be given within 30 minutes of receiving from blood bank. If it will not be used within 30 minutes return immediately to the blood bank
- Administer over 2 to 4 hours to prevent fluid overload. n
- Vital signs before (baseline), after 15 minutes and every hour until transfusion is completed n
- Jehovah’s Witness refuse blood n
- Start infusion at slow rate. Reaction occurs within 15 minutes or first 50 ml. n
- If reaction occurs, STOP TRANSFUSION IMMEDIATELY. Disconnect the tubing and run normal saline. n
- Send the blood bag and blood tubing to the blood bank. n
Acute Hemolytic Reaction
- Caused by mismatched blood
- Symptoms: fever, back pain, dark red urine due to hemoglobinuria and symptoms of shock (hypotension and tachycardia)
- Priority: Monitor kidney function (BUN, Creatinine and urine output) and IV fluidsu00a0
