  • Verify the client’s identifiers with another LICENSED Nurse.
  • Best IV: 18 gauge to prevent hemolysis
  • Only 0.9% NaCL can be infused with blood products
  • n
  • Must be given within 30 minutes of receiving from blood bank. If it will not be used within 30 minutes return immediately to the blood bank
  • Administer over 2 to 4 hours to prevent fluid overload.
  • n
  • Vital signs before (baseline), after 15 minutes and every hour until transfusion is completed
  • n
  • Jehovah’s Witness refuse blood
  • n
  • Start infusion at slow rate. Reaction occurs within 15 minutes or first 50 ml.
  • n
  • If reaction occurs, STOP TRANSFUSION IMMEDIATELY. Disconnect the tubing and run normal saline.  
  • n
  • Send the blood bag and blood tubing to the blood bank.

Acute Hemolytic Reaction

  • Caused by mismatched blood
  • Symptoms: fever, back pain, dark red urine due to hemoglobinuria and symptoms of shock (hypotension and tachycardia)
  • Priority: Monitor kidney function (BUN, Creatinine and urine output) and IV fluidsu00a0
